Abstract: Under certain circumstances, templated EHR notes can prove invaluable. When they are used to coordinate and facilitate follow up care, these notes can improve care delivery, simplify medication management and direct timely and appropriate follow up. By taking advantage of some intrinsic capabilities within the Computerized Patient Record System (CPRS), we created a templated note to facilitate care for ischemic stroke patients. In doing so, we incorporated current American Heart Association/American Stroke Association Guidelines, allowing providers to address all aspects of their patients transition from the inpatient to the outpatient setting.

Learning Objective 1: Creating a template to encompass all aspects of care for stroke patients to facilitate their transitions in care to include medications, education and appropriate appointment creation and follow-up.
